Simplify things by putting setup.h files themselves under version
control and getting rid of setup0.h ones.
The initial motivation for using separate setup0.h files was to allow
having local changes to setup.h, but with Git there is a simple way to
do it by using "git update-index --skip-worktree include/wx/msw/setup.h"
for example, so we don't really need setup0.h any more and dropping them
makes things simpler.
This doesn't seem to be actually necessary, i.e. the (implicit)
dependency on the output file is enough and using it resulted in always
redoing the custom build step with MSVS 2019 which seems to always
consider directories as missing dependencies, at least according to its
msbuild (version 16.1.76+g14b0a930a7) diagnostic build log.
Use the same format as is used by MSVS itself to avoid spurious changes when
modifying the files. Notably:
- Use DOS end of line format (0D 0A).
- Add UTF-8 BOM (EF BB BF) to the beginning of the files.
- Do not put CR LF at the end of the last line in the file.

Having CRLF files in the repository with eol=crlf breaks just about
everything, e.g. any rebase/merge involving these files would fail.
Try to fix this by making sure the files use LF in the repository and CRLF is
only used in the working copy.
Get rid of version-specific MSVS project files, we can reuse the same projects
for all MSBuild-based versions, i.e. 2010, 2011 and 2013.
