Override required functions in wxHeaderCtrlSimple to avoid asserts if
its columns are reordered.
Also bring the code behaviour in agreement with the documentation by
calling UpdateColumn() after calling UpdateColumnVisibility().
Finally, only call UpdateColumnsOrder() if the corresponding event was
not processed as the application should use one xor the other mechanism
for reacting to columns reordering, but not both.

This is a combination of running clang-tidy with modernize-use-nullptr
check for some ports (GTK, X11, OSX) and manual changes to the ports for
which it couldn't be used easily (MSW, DFB) and also manually updating
the docs.
Also replace NULL with null or nullptr in the comments as this is more
consistent with the use of nullptr in the code and makes it simpler to
grep for the remaining occurrences of NULL itself.
And also use null in the assert messages.
Only a few occurrences of "NULL" are still left in non-C files, mostly
corresponding to unclear comments or string output which it might not be
safe to change.
This makes it possible to get the appropriate column width from outside
the class, as GetColumn() itself is currently protected and so the
existing overload couldn't easily used.
In EVT_HEADER_RESIZING event handler there is necessary to update actual
column width and redraw the control to show the column with new width.

This keyword is not expanded by Git which means it's not replaced with the
correct revision value in the releases made using git-based scripts and it's
confusing to have lines with unexpanded "$Id$" in the released files. As
expanding them with Git is not that simple (it could be done with git archive
and export-subst attribute) and there are not many benefits in having them in
the first place, just remove all these lines.
If nothing else, this will make an eventual transition to Git simpler.

Use the same short names as are used by the event table macros for the event
type constants themselves. This makes them much more comfortable to use, e.g.
Bind(wxEVT_BUTTON) compared to Bind(wxEVT_COMMAND_BUTTON_CLICKED).
The old long names are still kept for backwards compatibility and shouldn't be
removed as it doesn't really cost anything to continue providing them, but all
new event types should only use the short versions.

Implicit size_t-to-int conversions provoke warnings when building 64 bit
version (in which sizeof(size_t) > sizeof(int)) with MSVC, so make them
explicit as the size_t values used here will always fit in int range.

number of columns in it, it turns out that getting column information from
the associated control is much easier than copying it into the control.
- Provide wxHeaderCtrlSimple derived class which can be used easily if
callback approach of wxHeaderCtrl is not needed.
- Because of wxHeaderCtrl virtualization, port-specific implementations of
wxHeaderColumn are not needed any more and were removed.
- Use wxHeaderCtrl in the generic wxDataViewCtrl: this means that column
events are broken right now in it as they haven't been implemented by
wxHeaderCtrl yet, this will be fixed a.s.a.p.
